Please send your medical record request from your Updox account. Please include your name, DOB, phone number, record you want to receive (last 3 years, all record, immunization record only, etc) and if you want to send it to yourself or your next PCP. $20 charge will be applied if the record is going to you and more than 20 pages.

Family Medicine & Lactation is led by Dr. Midori Nishimura. Since 1999, Dr. Midori has been treating patients of all ages, focusing on babies, children, and patients who need help with breastfeeding.
About Dr. Nishimura
Dr. Nishimura graduated from Stanford University School of Medicine in 1995. She completed her residency with San Jose Family Practice Residency Program in 1998. She was employed at Stanford Family Medicine from 1999 to 2007. In 2007, Dr. Midori opened her private practice in Mountain View, which was later moved to San Jose in 2017.

Dr. Nishimura was born and raised in Japan. She moved to the United States for college and has been living in the Bay Area since 1987. She is bilingual and bicultural and enjoys running, yoga, gardening, singing, and having a good time with family and friends. She is the finisher of 6 Marathons, most recently London Marathon in 2023.
